No specific form is required. The donor needs to write a Letter of Instructions to Fidelity, which should include:

 

Account From, number, name and tax lots of shares

Information from Vanguard with the receiving accounts and DTC number (see above)

 

              If the amount is under 10K: the signed and scanned letter can be uploaded via secure message to Fidelity

              If the amount is 10-30K: a phone call is required first

              If the amount is greater than 30K: medallion signature guarantee is required in addition to a phone call
